This form is for submitting hours for one session for 1-2 member (ex: spouses). You can also use the paper form, print it our here or you can always get a copy at the Lodge office.

If this is a group event (committee meeting, bulletin folding, fundraising event), PLEASE use the GROUP EVENT FORM here. 

If you have multiple volunteer sessions to submit, this is the wrong form!  Go to the Multi-Event Volunteer Form.
